Columbia Memorial Hospital's Diabetes . . . Take Charge Program has been recognized
by the American Diabetes Association for Quality Self-Management Education.
Our team consists of Certified Diabetes Educators, a Registered Dietician,
and a Registered Nurse. Diabetes Self-Management Education is the cornerstone
of care for all individuals with diabetes who want to achieve optimal health. Our
program is designed to meet your personal needs by providing you with the correct
information and skills to manage your diabetes. It is designed to put you in
charge of your diabetes and lead to successful outcomes.
