Medical Records

Your medical records are archived within the Medical Records Department at Columbia Memorial Hospital. If you wish for a copy to be sent to a physicians office or are requesting a copy for yourself, please call 518.828.8513.

Our Medical Records office is open Monday-Friday, 8:00am-4:00pm.
