for Participants
. Participants must be age 16 or older.
. Participants under the age of 16 are afforded an opportunity
to take part in a

structured
facility tour rather than a Job Shadow experience.
. Participants under 18 years of age require parent or legal
guardian signatures on
.
the Job Shadow Program Application, Organizational Ethics Form,
Student
.
Behavior
Form and Health Screening Questionnaire.
. Proof of two MMR immunization dates or proof
of positive Rubella and Rubeola
.
Titer
required.
Those
without appropriate signatures on required forms WILL
NOT take part in any programs.
Starting
the Job Shadow Process
There
are two easy steps to starting the job shadow process:

READ AND COMPLETE: |
Please
read the Guidelines for Participants and complete the following
forms: Job
Shadow Program Application, Organizational
Ethics Form, Student Behavior Form and Health Screening
Questionnaire. |
| SECOND
SUBMIT: |
Please
submit the following forms: Job Shadow Program Application,
Organizational Ethics Form, Student Behavior Form and Health
Questionnaire. Your appropriately signed Application,
Organizational Ethics Form, Student Behavior Form, and Health
Screening questionnaire must be received two weeks prior
to your arrival for the Job Shadow experience. Mail
