Community
Health
Because
each community served by Columbia Memorial Hospital has its own
unique set of health issues and because partnerships are so critical
in improving community health, we are committed to further identifying
community issues and working with local partners. Through a coordinated
approach to community service, Columbia Memorial Hospital’s
goal is to enhance our neighbor’s health status, community
by community.
During
the past year, Columbia Memorial Hospital enjoyed recognition
of its Community Health Program and commitment to excellence:
Diabetes
Take Charge Program.
Quit
Program
Columbia
Memorial Hospital identifies community needs by examining information
that pinpoints where services are needed, and by listening to
the individuals we serve. In fact, assessing the health
care needs for our community is an ongoing process at Columbia
Memorial Hospital. It involves every individual in our system – Board of Directors, associates, volunteers, and physicians
– and we reach out and ask community members themselves.
Meeting
Community Needs
In
2006 Columbia Memorial Hospital participated with various organizations
both public and private in an effort to address community needs.
Diabetes
Education: Diabetes Take Charge Program offers diabetes education
by a certified diabetic educator. The program educators worked
with physicians to coordinate a presentation on diabetes. In addition
the hospital actively participates in the Diabetes Coalition of
the Greater Capitol Region.
